CHANGE SIZE ON PICTURE WITHOUT STRETCHING: To ensure not stretching pictures when changing size. Hold Shift down on your keyboard and at the same time drag the corner of the picture. HOW TO CROP INSERTED PICTURES: Click the image. The tab Format, Picture Tools opens in the Ribbon. Click Format and the Crop icon appears in the Ribbon. UDFORDRINGEN Traditionelt har patientforløb bygget på, at en patient behandles hos én aktør og dernæst sendes videre til den næste aktør, med eftersendelse af journalinformationer For mange, især ældre patienter, er udtrykket ”den sammenhængende patientbehandling”, hvor flere aktører samtidigt arbejder sammen om en behandling, et mere dækkende udtryk for behovet. For disse patienter er der ikke længere tale om et antal afsluttende behandlinger og forløb, men snarere, at de har en løbende kontakt med flere forskellige aktører, der hjælper og behandler dem. Og hver aktør vedligeholder journaldata i eget system => de samme patientdata ligger i flere it- systemer => svært at få adgang til data Behovet for at have adgang til de korrekte og aktuelle data for patienten vil stige: Patienter udskrives tidligere fra hospitalerne og medfører øget behandling og pleje i den primære sektor. Mange ældre patienter behandles samtidigt for flere diagnoser og tager typisk 5 eller flere forskellige lægemidler. Patienter med kroniske lidelser belaster sundhedssystemet med mange kontakter til mange forskellige aktører. Hospitaler vil gerne øge andelen af patienter, der behandles ambulant. Telemedicinske løsninger vil øge antallet af patienter der ”udlægges” til eget hjem. 21/09/20163

Nødvendig med fælles it-services på tværs af sektorer (fælles it- systemer og infrastruktur) Empirisk forskning hos MIT/CISR i USA har vist at problemfri integration af processer (patientforløb) på tværs af aktører/ systemer forudsætter fælles it- services med mulighed for at dele data og transaktioner Alle store private koncerner, som ønsker en sammenhængende kundebetjening går/er gået samme vej En effektiv udnyttelse af de nye telemedicinske løsninger vil kræve fælles it-services på tværs af sektorer Indenfor sundhedsområdet i DK er FMK og Receptserver eksempler på fælles data og infrastruktur.
