Section of immigrant medicine, Department of Infectious Diseases, University Hospital Hvidovre ​ Competence Centre for Transcultural Psychiatry ​Mental Health Centre Ballerup ​Mental Health Services of the Capital Region of Denmark Migrants make up approx. 8% of the Danish population. Studies show that migrants have a higher risk of some mental disorders than the native population 1,2 Cultural awareness is important because of the risk of misdiagnosing mental disorders in migrants 3,4 Psychiatry in particular is faced with challenges due to language barriers and because the presentation, interpretation, understanding and treatment of mental health symptoms vary across cultures 1,2, 5 The Cultural Formulation Interview (CFI) in DSM-5 6 is a semistructured 16 question interview which is meant to ensure cultural competence in a clinical psychiatric setting. METHODS 1. Registry based study on diagnoses and diagnostic shifts among migrant patients Data from Statistics Denmark will include all migrants living in Denmark from 2003-2013 and a Danish-born comparison group (1:5). Data will be cross-linking with the Danish Psychiatric Central Register. The accumulated number of different diagnoses in the two groups will be compared over a period of 10 years, and the first and last diagnose in the given period will also be compared to give an estimate of diagnostic shifts in the two groups. 2. Observational study of the transcultural assessment and treatment at CTP All patients at CTP that meet the referral criteria and who are not enrolled in an ongoing randomised controlled trial at the centre will be included in the period from July 2014 to June 2016 - approximately 120 patients per year. The referral diagnosis will be revised at three different times: After a multidisciplinary assessment, between treatment module 1 and 2, and at the end of treatment. The diagnosis will be compared for each patient to evaluate whether diagnostic shifts have occurred. 3. Study on the use of the Cultural Formulation Interview (CFI) 70 migrant patients referred to the following clinics will be included from January to June 2015: CTP, Section of immigrant medicine, Department of Infectious Diseases, University Hospital Hvidovre and DPC Ishøj. A CFI will be carried out by a doctor and the doctor will note if the information from the CFI changes his or her view on the understanding of the patients’ symptoms, diagnoses, or the content of the treatment plan. The data collected will be analysed to give a quantitative estimate of the additional information obtained by using the CFI. This study is supported by Mental Health Centre Ballerup. Author for correspondence: Signe Skammeritz, MD, PhD student signe.lund.skammeritz@regionh.dk // www.ctp-net.dk Competence Centre for Transcultural Psychiatry BACKGROUND IMPACT 1.What is the prevalence of psychiatric diagnoses among migrants compared to native Danes and do migrants have a higher number of diagnostic shifts compared to native Danes? 2.Do migrant patients change diagnoses during the course of referral, assessment and evaluation of the treatment at the Competence Centre for Transcultural Psychiatry (CTP) and if so, what are the possible causes for the changes? 3.Do clinicians find that the use of the Cultural Formulation Interview (CFI) provides important information for the diagnostic process and treatment plan? 4. AIM AND RESEARCH QUESTIONS The overall aim of this study is to strengthen the knowledge of the assessment and diagnoses of migrant patients with mental health problems in Denmark and thereby increase the possibility of improving the psychiatric care for this group of patients. This PhD project consists of three sub-studies, with the following research questions: 1. Provides the necessary knowledge for developing a clinical procedure for a more precise and effective assessment and treatment plan for migrants with mental illness, and: Thereby increases the patient’s acceptance of and involvement in the treatment. Ensures qualified and evidence-based treatment for patients with a migrant background. Involves different outpatient mental health services, involves both clinicians and patients. The broad recruitment will ensure that results are representative in a larger context in the Danish health care system. Provides much needed evidence to the international research field on the clinical utility of the CFI, in cultural encounters. socio-economic benefits. Thus, this research project can have a high impact on the mental wellbeing of the patients, the everyday clinical life of health care professionals and on the health care system and Danish society as a whole. 1.
